Once you have printed or received your self-assessment form, simply fill it in, sign it, and send it back to us through the post.

Help and Information!
Every completed form will be entered into our monthly prize draw for £25 worth of gift vouchers. Please attempt to answer all the questions; some have a 'don't know' option. The information will be used to assess the energy efficiency of the housing stock in your area.

We require the following information from you to process your application:

  • Name and address (including postcode)
  • Signature of the person completing the form
  • Property and central heating details
  • Which benefit you are receiving (if applicable)
  • If you are over 70 years old (required for over 70's scheme)
  • Household Income (if you feel you are classed as 'fuel poor')
Failure to provide this information could prevent you from receiving a free grant towards loft and cavity wall insulation.
